Quinton Griffith placed the visitors in the lead in the 20' with a header inside the box on a free kick opportunity.
Joseph Willinx tied the score in the 45' from a penalty kick conversion, placing the ball inside of the left post.
Willinx earned his brace in the 69' with another conversion from the penalty marker.
Malcolm Stewart struck inside the penalty area in the 89' to tie the match and rescue a point for the Antigua and Barbuda side.
The late equalizer marks a second consecutive for Grenades after tying in the 90’+5 of their match against AC Port of Spain one week ago.
As a result, Moca FC secured one of the top two spots in the group and will advance to the semifinals.
Grenades FC and Ouanaminthe FC will conclude their group stage participation next Tuesday against Moca FC and Cibao FC, respectively.
